PRINCIPAIS COMPLICAÇÕES E EFEITOS COLATERAIS PÓS-TRANSPLANTE RENAL
Abstract
A Insuficiência Renal Crônica ocorre pela perda progressiva
dos rins, quase sempre irreversível, e a melhor alternativa
é a Terapia Renal substitutiva, que requer por meio de
diálise, hemodiálise ou transplante renal. O transplante
renal constitui o melhor tratamento, permitindo maior
sobrevida do paciente. Apesar de inúmeros benefícios, apresenta
várias complicações relacionada a cirurgia, a patologias
entre doador e receptor e aos imunossupressores
que são administrados por toda a vida para minimizar as
chances de rejeição. A mortalidade ainda é relevante no
primeiro ano após o transplante, decorrente de complicações
cardiovasculares e infecções, no qual as bacterianas
constituem a principal causa de infecção no primeiro mês,
após surgem as infecções virais e fúngicas, entre as mais
acometidas infecções respiratórias e urinárias, e em seguida
ocorrem as infecções virais, em especial por Citomegalovírus.
Os imunossupressores frequentemente são utilizados
para doenças auto-imunes e transplante, beneficiando ainda
a sobreviada do enxerto a curto prazo e diminuindo os
efeitos secundários. Mas apesar de apresentar grandes benefícios,
possui muitos efeitos colaterais, como nefrotoxicidade,
hipertensão, hiperlipidemia, diabetes mellitus, entre
outros. Através de uma revisão bibliográfica, apresentamos
as principais complicações em pacientes submetidos ao
transplante renal e os efeitos colaterais provenientes de
medicamentos imunossupressores.
