A agenesia de veia cava inferior é uma anomalia congênita muito rara, que a poucos anos foi identificada como um importante fator de risco para o desenvolvimento e a recorrência de trombose venosa profunda de membros inferiores em pacientes jovens. O objetivo desse trabalho foi relatar o caso de um paciente jovem de 21 anos de idade do sexo masculino, que apresentou um quadro de trombose venosa profunda de membros inferiores. A tomografia computadorizada demonstrou ausência de veia cava inferior e a ultrassografia com Doppler venoso colorido obteve um diagnóstico de Sinais de trombose venosa profunda bilateralmente.
