Associação glibenclamida/metformina no tratamento do Diabetes Mellitus Tipo 2

Diversas estratégias terapêuticas tem sido propostas para alcançar um
adequado controle metabólico no diabetes mellitus tipo 2. O objetivo do
tratamento do diabetes mellitus tipo 2 deve ser obter um controle glicêmico
satisfatório: glicemia de jejum (GJ) < 100 mg/dl, glicemia pos-prandial
(GPP) < 140 mg/dl e hemoglobina glicosilada A1c (HbA1c) 7). As
sulfoniluréias atuam desencadeando a liberação de insulina a partir das
células β pancreáticas enquanto as biguanidas diminuem a insulino
resistência. A perda de um adequado controle glicêmico após um período
inicial variável de tratamento com sulfoniluréia ou biguanida abre a
possibilidade da associação destes fármacos. A associação de
sulfoniluréias e biguanidas, em particular glibenclamida e metformina
melhora significativamente o controle glicêmico considerando que a GJ,
GPP e HbA1c se reduzem significativamente com a associação destes
antidiabéticos orais.
